This is a blatant lie. No dealership is going to let you take a vehicle without registering it. They're legally obligated to register the vehicle, and they do all of that for you when you purchase it. That's part of what that document fee you pay goes towards. So I figure the rest of the rant is probably ignorant storytelling too.

Brand new off the lot could easilly mean old as shit, paid in cash from a shady dealer, probably with a salvage title or similar.

That would be "used, off the lot", but I guess these people probably aren't masters of accurate communication. Even those shady dealers aren't likely to risk their vehicle sales licenses over something stupid like this which is totally routine for them.

Am I correct in assuming it was a new to her truck, likely purchased in a private sale? Or are dealerships in other states not required to fill out registration paperwork?

She said she got it off the lot. I figure the whole thing is a fantasy scenario they invented. You can't take a vehicle from a dealership without it being registered. No dealership is going to risk their vehicle sales license over one deal with a crazy person.
