I guess you have to consider what is important to you. I am 10 years hrt with half of that post orchi, I am able to get an erection and still have ejaculate when i orgasm, however if I wanted to top I would need to use something like Viagra. As long as you are willing to get an erection you shouldn't have atrophy, I made the choice to let myself atrophy for about a year and when I did get an erection it was horrificly painful. Use it or lose it is a valid phrase in this case.
Getting semi-frequent blood tests and talking with your doctor to get a dosage that works for you in the long term is probably the best approach.

surgery talk
Continuing my zero-depth bottom surgery research and realize that some are asking for psychiatrist letters that I no longer have access to and the mental health professionals I saw during my early transition are no longer in practice. This combined with the fact I'm in a different country than the one I got the letters for my orchi and the actual procedure are kinda devastating, not really an option to get new letters in the country I'm living in and there is no way I can return to the country I moved from for a long enough duration to get new letters from a new psychiatrist. I guess I need to budget a lot more, so I can pay for a private doctor in another country that won't ask for this. It is so frustrating to have been transitioning for 10 years and had other procedures done 5 years ago and still have to fucking jump through hoops. I think I read somewhere that some doctors in Thailand don't ask for the letter or have a psychiatrist on staff to provide it but idk this unexpected roadblock kinda fucked my desire to research more.
Fuck it has taken years to mentally recover from the trauma i experienced with an earlier surgery and now when I'm ready to try for vaginoplasty and the evergreen trans experience of being gatekept occurs again.
