Sorry my gender has been really boring the past few weeks, everyone lea-sad

Uh cutting my cyproterone acetate dose down to 12.5mg has been rad at least, did that a couple weeks ago now...

[–] [email protected] 6 points 1 month ago (1 children)

Ash you are a magnificently powerful poster. I don’t think you could be boring if you tried. cat-trans

[–] [email protected] 6 points 1 month ago (1 children)

Awwwwwh no toriel-blush

Idk, sometimes I worry about spamming the mega with too many gender-unrelated things. Like Wayhaven you can play as an enby so it counts, but I have not made a Genderpost really since I got skirtpilled...

[–] [email protected] 5 points 1 month ago* (last edited 1 month ago) (2 children)

I contend that every post is a gender post, cis or trans, and we must vastly increase the number of transgender posts to down the cis. theory-gary

[–] [email protected] 4 points 1 month ago

this i wouldn't be posting if this thread didn't veer massively off topic all the time, i feel at home when i see Sonic posts or Wayhaven stuff

[–] [email protected] 5 points 1 month ago (1 children)

cypro is busted, I cut to 12.5 every 2 days a while ago and still have next to zero T. would honestly like a little more lol, maybe I should cut it even further.

[–] [email protected] 6 points 1 month ago (1 children)

I would like to do that too, I worry that cypro every other day would mess with my mood though...

Under socialism cypro tabs will be 25mg and split four ways easy into 6.25mg or whatever.

[–] [email protected] 4 points 1 month ago* (last edited 1 month ago) (2 children)

I'm fairly sure because of its long ass half life 12.5 every other day is more or less equivalent to 6.25 daily! I haven't noticed any mood or energy issues since switching:)

[–] [email protected] 5 points 1 month ago (1 children)

Okay cool, good news, perhaps I will try it sometime!

[–] [email protected] 4 points 1 month ago (1 children)

you may have seen this before but this is what informed my decision: https://transfemscience.org/articles/cpa-dosage/

[–] [email protected] 5 points 1 month ago (1 children)

and at doses of 50 to 300 mg/day as an antiandrogen.

Uh I think that entire range is irl crimes madeline-shock

the more as some of the side effects may be related to the progestational overdosage rather than to the administered antiandrogenic activity. […] Adverse reactions like tiredness, lassitude, and increase in body weight are possibly due to the enormous overdose of progestational activity in the formula

Well that fuckin makes sense, and these studies are from the 1970s, ffs. Western medicine is a joke. What interested me here though

T decreased from ~431 ng/dL at baseline to ~149 ng/dL with CPA (–65%) for the 10 and 20 mg/day doses combined.

Seeing figures like this make me go Oh, well that's not like upwards of 90%! But as stated in the article E, even at like 4mg would offer a pretty good amount of suppression too though, these dorks are cypro only:

Fung and colleagues showed that the combination of either 25 or 50 mg/day CPA with a moderate dosage of oral estradiol (~3.5 mg/day) or transdermal estradiol (~3.5 mg/day gel or ~100 ΞΌg/day patch) resulted in equivalent and complete suppression of gonadal testosterone production (~95% suppression of testosterone levels) in transfeminine people (Fung, Hellstern-Layefsky, & Lega, 2017).

So CPA's not 100% effective on its own under 50mg, but more than like 10mg is overkill with estrogen:

Accordingly, Meyer et al. (2020) assessed a dosage of CPA in combination with estradiol in 155 transfeminine people and found no difference in testosterone levels with 10, 25, or 50 mg/day CPA; testosterone levels were strongly suppressed with all three doses (to about 15–20 ng/dL on average, or into the lower end of the normal female range). The estradiol forms and doses used in this study were oral estradiol valerate (median 6 mg/day, range 3–10 mg/day), transdermal estradiol gel (median 2.25 mg/day, range 1.5–6 mg/day), and transdermal estradiol patches (100 ΞΌg/day in all cases). Estradiol levels were about 100 pg/mL on average, with an interquartile range (i.e., difference between 75th and 25th percentiles) of about 100 pg/mL. This study demonstrates that, provided estradiol levels are adequate, no more than 10 mg/day CPA is needed to fully suppress testosterone levels in transfeminine people.

I think I'll wait to do 6mg though, I dunno:

Even doses of CPA lower than 5 mg/day (e.g., 2 mg/day) may be usefully effective for testosterone suppression if combined with sufficient levels of estradiol, although this has not been studied and remains to be validated. But there is certainly precedent for the notion when looking at studies with other progestogens.

I don't see a lot of support for lower than 10mg, Idk. I do wish this shit came in 10 or 20mg tabs instead of fucking 50.

[–] [email protected] 4 points 1 month ago* (last edited 1 month ago) (1 children)

yeah it's a pain. I guess it can vary from person to person, my choice to go to 6 was also informed by anecdotal accounts on bsky of its effectiveness at that dose, and for me my T is perfectly acceptable at 6.25 along with my estrogel. I didn't really experience the increased energy and stuff that some folks report with the lower dose, but relying on less of a drug is just nice. also yeah it's off the wall that we used to get prescribed more than 50mg, that's gotta do some damage.

[–] [email protected] 6 points 1 month ago (1 children)

It worries me now that I took 50mg for like a few years, guess it could have been worse but dang... relying on less of a drug is cool, I'll see Idk. I'm not really smart enough for all this chemist shit sadly, much as I would adore becoming a trans Doctor Without Borders or smth.

[–] [email protected] 4 points 1 month ago* (last edited 1 month ago) (1 children)

what's nice is that i was able to stockpile it for a while and then one of my gfs was able to switch to it from spiro without needing a script, I get enough from the pharmacy for both of us hehe. and I wouldn't worry, 50mg may be too much for hrt but it shouldn't have done any harm from what I know if you were presumably on estrogen at the same time.

[–] [email protected] 5 points 1 month ago

waow-based CPA stockpile...

Yeah I was on 4mg lupin-estradiol the whole time, so that's good...

[–] [email protected] 2 points 1 month ago

Yeah my doctor told me I could take the whole 50mg pill every 4 days if I wanted to

[–] [email protected] 5 points 1 month ago (3 children)

damn i keep forgetting to talk to my doctor about that i am still on spiro, were you on spiro before? is cypro way better?

[–] [email protected] 6 points 1 month ago (2 children)

cypro is a significantly more effective T blocker with less side effects and will be preferable in the majority of cases. it's also a progestogen iirc which is cool?

[–] [email protected] 5 points 1 month ago (3 children)

Why do they prescribe spiro then? I feel like I see a lot of people starting on spiro.

[–] [email protected] 5 points 1 month ago (2 children)

CPA is fuckin illegal or some shit in the burger reich for some goddamn reason. Fucking FDA bullshit probably.

[–] [email protected] 4 points 1 month ago (1 children)

oh, i had no idea, is it legal in canada?

[–] [email protected] 5 points 1 month ago (1 children)

Yeah I'm not doing crimes by eating it, lol. It's legal like everywhere else in the world, just not in the joke burger failstate :3

[–] [email protected] 4 points 1 month ago (1 children)

so maybe I should talk to doctor then, but then i have to make a phone call to the very nice people at my clinic (they are very sweet) and see my very nice doctor (i am very lucky, my last doctor quit and there's real problem with docs in my "city"

Joke Burger Failstate

beautiful word rhythm, as always. i prefer to come up with phrases for people and things when i'm talking about them that describe my connections to them, this is a good example lol

[–] [email protected] 5 points 1 month ago

Phone calls are shit but that's what I did, was talked to my endo and got switched, it's rad.

:3 Yeah it slaps, I'm big into the "burgerreich" term originating from here as well

[–] [email protected] 3 points 1 month ago (1 children)

The purported reason it is illegal in amerikkka is that cpa has a very rare probability of creating non malignant brain tumours.

[–] [email protected] 4 points 1 month ago* (last edited 1 month ago) (1 children)

cpa isn't FDA approved I think. I'm in EU where it's much more common than spiro and it is just better by the current literature. online english spaces tend to be US centric so you see a lot of spiro talk.

[–] [email protected] 4 points 1 month ago

Oh nice yea I love america.

[–] [email protected] 2 points 1 month ago

The company that makes it literally cannot be bothered to apply to get it approved by the FDA so doctors aren’t allowed to prescribe it

[–] [email protected] 3 points 1 month ago

oh jeez i gotta get in on this, save me some

[–] [email protected] 5 points 1 month ago (2 children)

Yes I was on spiro :3 I peed a lot and sort of desired the "depressive" effects of cypro since I have extremely poor emotional regulation, but 50mg is too much beyond a temp dose usually. Even on 12.5mg my T levels are almost nothing, which fucks. It's good at what it does, but whatever antiandrogen method works best is what's best for you. If your levels come out well and the side effects of spiro (mostly just pee Idk) are fine by you, dw abt it tbh.

[–] [email protected] 5 points 1 month ago (2 children)

but 50mg is too much beyond a temp dose usually

you mean on cypro, right? i'm on 300mg spiro...

Even on 12.5mg my T levels are almost nothing, which fucks

ok yeah i follow now

If your levels come out well and the side effects of spiro (mostly just pee Idk) are fine by you, dw abt it tbh.

yeah i'm planning to get bottom surgery (or at least I think I am? oh man that's a whole thing right now that I'm trying to process in small bursts) so I was just gonna go off anti-andros after that

[–] [email protected] 5 points 1 month ago (2 children)

Yeah I mean cypro, which comes in 50mg tabs but should AT LEAST be divvied into 25mg halves, and I've seen as low as like 6mg. Spiro, anything from 150mg to like, 400mg? is normal.

The bottom surgery conundrum bocchi-head-shake going off antiandros is pretty based, good for your kidneys and/or liver.

[–] [email protected] 4 points 1 month ago (1 children)

yeah, this is why i need to Perfectly Understand My Gender As Soon As Possible, so that I know I'm making the right decision.

[–] [email protected] 5 points 1 month ago (1 children)

kel-what But surely there's no pressure, and you can take your time deciding how you feel about it, right?

[–] [email protected] 5 points 1 month ago (1 children)

yeah i decided on the 1-3 year option in a nearby city (that i used to live in lol, so at least i know it well) instead of flying across the country for the less than a year option, which came with a bunch of caveats like i have to get electrolysis done etc.

...this post is an opsec failure

[–] [email protected] 5 points 1 month ago (2 children)

I couldn't dox you fwiw ✨ also caveats suck, death to the medicalisation of trans people under this system.

[–] [email protected] 4 points 1 month ago (1 children)

Thats a relief that at least (although I'm not concerned about anyone posting in bear site in this regard, come visit, but it IS a public forum), but it will be nice to be nearby and not have to fly for the appointments and surgery, even if it does take longer and I have to get hair removal.

[–] [email protected] 5 points 1 month ago (1 children)

Yeah it always sounded kind of harrowing to fly for stuff...

[–] [email protected] 4 points 1 month ago

Yes especially after surgery ugh.

[–] [email protected] 4 points 1 month ago (1 children)

Oh but I guess at least I dont have to pay anything! That's an improvement over years ago when I started transition

[–] [email protected] 5 points 1 month ago

Free is extremely cool, yeah. That's awesome.

[–] [email protected] 4 points 1 month ago

yeah totally, that's one of my motivations, being able to cut back on meds

[–] [email protected] 4 points 1 month ago

I'm no where near bottom surgery but it really is a whole thing meow-hug

[–] [email protected] 2 points 1 month ago

Woof I had to stop spiro because the brain fog was so bad that it was hazardous for me to drive. So glad I’m just on monotherapy now.

[–] [email protected] 2 points 1 month ago

I was on spiro briefly before switching to cypro. I had the excessive peeing, but mainly it just made me really dizzy. Cypro is way better

[–] [email protected] 2 points 1 month ago

I just switched down from 12.5 mg daily to 6.25 mg daily (12.5 every other day). I haven’t got my levels checked yet, and it’s probably fine, but I’m still worried it’s gonna make my T levels go up. Hopefully being on a lower dose makes the prog less depressive
